We’re introducing a new and improved request form for our medical and social canvassing services. Based on feedback from our customers, this intuitive new form makes it easier than ever to quickly request the services you need. Here’s an overview of what you can expect in the ordering process:
1. Medical Canvassing
First, choose between our QuickTEL Canvass Express Bundle or Canvass By Location Type offerings. With the QuickTEL Canvass Express Bundle, you will receive medical checks at 20 facilities (10 hospitals and 10 pharmacies) in a 70-mile radius of the address you provide within 48 hours.
Our Canvass By Location Type is an a la carte offering that enables you to select as many provider types as you want including hospitals, dental offices, physical therapy, gym facilities, and so much more. These results have a 5–7 day turnaround time.
You can choose a QuickTEL Canvass Express Bundle and add additional locations to be canvassed in the a la carte option but note that the a la carte locations will have a 5-7 day turn around.
*Please note: If you’re only interested in requesting a social canvass, you can skip this section all together.
2. Records Retrieval
Following your medical canvass selection, you can choose to request to retrieve the records found during the canvass. This includes medical and billing records, employment records, phone records and more. Easily upload your Medical Authorization or Letter of Representation by dragging and dropping into the form.
3. Social Canvassing
Next, you can select one of our social media investigation services, Social e-Profiler or Social Canvassing. Both of these options search multiple social media platforms such as Facebook, Instagram and Venmo for actionable data and deliver the findings in a PDF Export. If you select Social Canvassing, you will also receive a review and findings summary created by our licensed investigators.

4. Order Review Page
Finally, after you enter your information and the claimant’s information, you will be taken to our all-new Order Review page. The Order Review gives you the opportunity to see the details of your request clearly listed so you can move forward with full understanding of your order or make corrections to the information you entered before proceeding.
